Athlete Medical Form & Athlete Participation Waiver

Athletes must submit a completed Medical Form and Participation Waiver before participating in Special Olympics and every 3 years thereafter. Athletes in our Young Athletes Program (ages 3-7) complete a different form available from our YAP coaches.

If an athlete’s application elects to refuse medical care (see item 4 on the last page of the Athlete Medical), then the athlete must also complete one of these forms:

If a pre-participation examination finds symptoms of spinal cord compression or atlanto-axial instability, and a doctor provides clearance for participation following a neurological examination, also complete the following form:

Volunteer & Unified Partner Application

All volunteers and Unified Partners, including minors, must submit a completed volunteer application when starting with Special Olympics and every 3 years thereafter.

Communicable Disease Waiver

All Special Olympics participants who attend any Special Olympics training or competition event must complete and submit the Communicable Disease Waiver (CDW) BEFORE participating. Participants include athletes, volunteers, coaches, Unified Partners, counselors, parents, and anyone else attending a Special Olympics training or competition event.

Email completed CDW forms as a PDF file to medical@somdmontgomery.org.

The CDW form replaces our previous Covid-19 Acknowledgement of Risk (AOR) Form. All athletes and volunteers must complete the new CDW form even if you previously submitted your AOR form. Eventually, Special Olympics will incorporate this waiver into the overall waiver that athletes complete with medicals and that volunteers complete with their volunteer application.

CVS Minute Clinic Special Olympics Voucher

Special Olympics Maryland has arranged for CVS Minute Clinics to perform an athlete physical at a discounted rate of $66.93. Present the voucher below to a CVS Minute Clinic to take advantage of this offer.

BEST PRACTICE TIP: Bring a Special Olympics Athlete Medical Application & Waiver Form with you to CVS and ask the clinician to use that form. If you use the Special Olympics form offered by CVS Minute Clinics, we will accept it but only if you also separately complete and submit page 1 of the Athlete Medical Information & Waiver Form. The CVS form does not include the information on page 1 of our form.
